Terms and Conditions
1. I have thoroughly read all Audition information online and understand all of the stated policies, fees, and requirements of participation.
2. I read and understand all "check box" items at the top of this audition form.
3. I will clear all school, family, and work conflicts during Tech Week and performance weekend.
4. I (parent or guardian) will meet all volunteer requirements (or discuss other arrangements).
5. I understand and agree that rehearsing for and dancing in The Nutcracker is at my own risk, or at my child's / children's own risk, and that Garden City Ballet, its Board of Directors, Artistic Team and staff, and volunteers are not responsible for any injury that might occur.
6. I give permission for the minor named on this form (if under the age of 18) to audition for, attend rehearsals, and perform in The Nutcracker.

After submitting the registration form online, a confirmation will be emailed to the registrant and parent contact (1) email address.
